New Medical-Related Absences request form in Vector
Employees can now request medical-related absences including disability management services, by submitting a Medical-Related Absences request form in Vector. The form does not ask for or allow personal medical information to be entered. All medical information must be faxed, mailed or hand delivered to the Health and Employee Wellness (HEW) clinic.
- Request time off due to illness or injury
- Request time off to care for a family member or partner who is ill, or to care for a newborn
- Request time off for medical appointments not related to an illness
- Links to policies and procedures detailing medical-related leaves, including sick leave, Family Medical Leave Act, workers’ compensation and parental leave
The laboratory offers many types of paid and non-paid absence benefits which can be confusing for employees and managers to implement. Argonne’s Disability Management (DM) program helps employees navigate absences involving medical care, assists in return to work, facilitates coordination with divisions concerning work restrictions or accommodation requests, and administers workers’ compensation benefits for work-related injuries and illnesses.
Prior to the Medical-Related Absences request form, requests for guidance came to HEW through phones calls, emails and walk-ins. The Vector form provides an easy way for employees to initiate medical-related absence requests and helps HEW prioritize those requests.
All requests will receive a response within two working days.
